Hospital systems, rehabilitation facilities, and physician offices trust Interim HealthCare of West Michigan because of our compassionate approach, thorough communication, quick startups, and ability to offer multiple levels of care under one roof. Our local care liaisons work closely with discharge planners, physicians, and families to ensure services are coordinated before discharge occurs—helping make the transition home as smooth and stress-free as possible.

We provide a full continuum of care, including caregiver services, personal care, companion care, respite care, dementia care, Alzheimer's care, skilled home health (RN, PT, OT, SLP, MSW, HHA), rehabilitation therapy, hospice care, and Veteran care services. As your needs change, your care can change with you.

One of the biggest concerns families have is, "What happens when I get home?" In many cases, caregiver services can begin the same day you discharge, with a trained caregiver ready to assist upon arrival home. Caregivers can provide short visits, up to around-the-clock 24/7 care! Skilled home health services are then coordinated to begin shortly after discharge, ensuring continued support and recovery with our clinical team.

Did you know? Michigan patients have the legal right to choose which home health or hospice agency receives their referral. Before discharge, talk with your care manager about your options and select the agency that best fits your goals, preferences, and care needs.

New, No-Cost Support for Dementia Caregivers: The Medicare GUIDE Program

Supporting Families Living with Dementia & Alzheimer's

A dementia or Alzheimer's diagnosis can change everything—not just for the person diagnosed, but for the family members caring for them. If you're feeling overwhelmed, you're not alone.

Through Medicare's GUIDE (Guiding an Improved Dementia Experience) Program, eligible families can receive additional support, education, care coordination, and now up to $2,625 annually in respite care benefits.

How We Help

At Interim HealthCare, we make the process simple. Steps:

  • Call us to be screened for GUIDE eligibility
  • If eligible, we will connect you to our GUIDE partner to start the enrollment process, and coordinate a no-cost in-home assessment. If not eligible, we will inform you of all your other care options - we won't leave you hanging!
  • Connect you to 24/7 care management support which provides education on dementia progression, communication strategies, fall prevention, wandering risks, and changing behavior
  • Provide trusted respite care through our experienced caregivers. You get to choose the schedule that meets your families needs!

Family caregivers need support, too. The GUIDE Program helps you access the resources, education, and respite services you need to continue caring for your loved one while prioritizing your own well-being.
