Case Management Services in Ocean County, NJ

Navigating today's healh care maze on behalf of a loved one can be an intimidating and frustrating process. Coordinating between different points of care delivery (doctors, hospitals, rehab, specialists, etc.), managing medications, maintaining a safe and comfortable home environment, and ensuring proper nutrition all add up to a complex process that can require a significant time commitment. These issues are often magnified if your loved one lives a long distance from you, and/or lives alone with no immediate support network.

Our Case Management Services consist of a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the patient's needs. Our case managers are Registered Nurses (RN's) who work with patients, their families, and other health care professionals to customize a plan that meets the unique needs of each individual. Our nurse case managers work hard around the clock to advocate on behalf of their patient/client, with their ultimate goal to achieve client wellness and autonomy.

The case management process embraces the concepts of advocacy and communication, and facilitates care along a continuum through effective resource coordination. Our nurses accompany their patients to doctors appointments, visit them during stays in the hospital, and help coordinate their discharge from a rehab facillity to ensure a safe and seamless transition to the home.  They conduct personalized visits to their clients on a previously agreed upon schedule and follow up all visits with detailed written reports and phone calls to the patient's family, guardian, etc. The goals of case management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient's right to self-determination.

I want to express my appreciation and gratitude for your help with my Mom. She has grown very close to a couple of your care givers. Everyone is prompt and considerate of her needs. It’s comforting to know I have people who I can count on.
