Our Case Management Services consist of a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the patient's needs. Our case managers are Registered Nurses (RN's) who work with patients, their families, and other health care professionals to customize a plan that meets the unique needs of each individual. Our nurse case managers work hard around the clock to advocate on behalf of their patient/client, with their ultimate goal to achieve client wellness and autonomy.
The case management process embraces the concepts of advocacy and communication, and facilitates care along a continuum through effective resource coordination. Our nurses accompany their patients to doctors appointments, visit them during stays in the hospital, and help coordinate their discharge from a rehab facillity to ensure a safe and seamless transition to the home. They conduct personalized visits to their clients on a previously agreed upon schedule and follow up all visits with detailed written reports and phone calls to the patient's family, guardian, etc. The goals of case management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient's right to self-determination.
