Personal Care & Support

Many older adults prefer to stay at home as they age

But is this the right choice for you or your loved one? The goal of home care (or assisted living) is to help the elderly remain at home as long as they are able, rather than moving into a long–term care facility.

Personal care and support services may be the perfect choice for those in need of minor assistance with daily activities such as: bathing, dressing, light housekeeping, meal preparation, and medication reminders. If a senior doesn't have a close network of nearby family or friends around to help, personal care and support is essential to keep them safe, comfortable, and independent within their home.

Our personal care assistance could also be the extra support someone needs as they are recuperating from a stay at a hospital or rehabilitative facility.  In each instance, our trained Care Professionals are dedicated to providing quality care with compassion and a commitment to the highest level of customer service.

Chronic Disease Management

INTERPath is Interim HealthCare's umbrella for our programs that have established clinical pathways and processes, including patients with chronic diseases, enabling us to work with physicians; while helping their patients learn to independently manage at home.

INTERPath supports the successful management of chronic conditions by:

  • Empowering patients to take charge of their condition
  • Helping patients (and their families) understand what events contribute to making their symptoms more serious
  • Providing guidance on how to manage unwanted events and improve their quality of life
  • Reducing the number and/or frequency of unwanted events
  • Providing instructions on how to manage medications
  • Avoiding hospital readmissions, emergency room visits and even unnecessary physician office visits
  • Enabling patients to stay independently at home

INTERPath rests on a comprehensive platform that integrates all the important elements of successful chronic disease management:

  • Specially trained, multidisciplinary care team
  • Best practice pathways for care and patient teaching and motivation
  • Patient teaching and self-care tools, including DVDs/videos
  • Technology in care planning and delivery of patient self-management
