Local Healthcare Company Announces their innovative Interim Transitions® Program aimed at providing chronic care support to help reduce hospital readmission rates.

LEXINGTON, MA – September 20, 2015 – Interim HealthCare of Lexington MA announces their innovative program “Interim Transitions®” aimed at helping hospitals improve care for patients transitioning home with the goal of reducing readmissions.

The Interim Transitions program is significant in that the Affordable Care Act penalizes hospitals that have “excessive” readmissions of patients who are admitted with heart attacks, heart failure or pneumonia and return with a readmission 30 days of their initial discharge. Interim Transitions offers hospitals solutions to better help their patients adhere to their post-discharge care plan and also to provide the support the patient needs to recover at home. 

With a long standing history of clinical expertise in home care, Interim understands the difficulties patients, their physicians and hospitals face with chronic diseases. While some readmissions cannot be avoided, many occur because the patient, while well enough to be discharged, still needs assistance returning to normal activities and in some cases understanding all the medications they must take.

Key factors of the Interim Transition program include; assisting discharged patients with transportation services to get to their follow up appointments as well as medication reconciliation and reminders to ensure they are taking the correct medications and doses at the right time. Interim Care Professionals are trained to help patients with symptom monitoring to catch warning signs that the patient may be experiencing changes in their health. Changes can trigger a call or appointment with the client’s physician before it gets to the point of readmission. As part of the program, patients are given a personal record book to help them monitor their health status. 

Other services can include assistance with daily activities such as bathing, grooming, meal preparation, shopping and even light housekeeping. While the need for these services is typically not covered by Medicare, Medicaid or insurance, most individuals understand the value and opt to pay privately to ensure their loved one is properly cared for as they return home after a hospital stay.
