What Services will Home Health Care Provide?
Home Health services are quite specific, but can be organized into three categories:
EDUCATION:
The goal of home health professionals is to equip the patient and family to manage their condition safely and effectively on their own. Education is a key part, and includes:

Understanding your disease or condition.

Understanding what makes it worse or better and how to recognize when things are getting worse.

Knowing what to do when your condition worsens: What medications to take, what activity to do or to avoid, when to call the doctor, when to go to the hospital

NURSING INTERVENTIONS:
These are services that require the skills of a nurse to complete.

Assessments and planning. The nurse completes a comprehensive assessment of
you and your condition and uses that to establish, with your physician, a plan of care

Taking vital signs readings: blood pressure, heart and respiration rates

Administering some medications such as injections or infusions

How do I Access Interim’s Home Health Care Services?

Referrals: Most home health services, because they are considered by regulation to be skilled health care services, require an “order” from a physician. But, anyone can call to ask about how home health might help them: prospective patients, family, advisors, neighbors, and staff in an assisted living facility – anyone. Interim HealthCare will take care of securing the physician order.

Payors: Medicare and most insurance companies have some benefit that cover home health services and Interim HealthCare participates in most every provider network. We will work with you to determine what’s covered and what the requirements for coverage might be for your Medicare or insurance coverage.
There usually are some requirements by Medicare and most insurance plans to access coverage for your home health services: These typically include:
· Homebound
Medicare and most insurance plans won’t pay for home health services if you could reasonably get to the doctor’s office or a health care facility to receive services. But this doesn’t mean you must be confined to bed. You could even have to go out for health care appointments or even to church and still be considered to be homebound. · Skilled Need:
Medicare and most insurance plans pay only for skilled nursing and therapy services. They typically do not pay for custodial care.
· Intermittent:
Medicare and most insurance plans pay for services to assist you recover safely at home. They don’t pay for around the clock nursing care.
