HOME HEALTH CARE

Skilled Nursing
Registered Nurses provide services such as health status assessments, education on disease management and a variety of treatments for clients, including: sampling for lab tests; wound care, medication training and compliance monitoring, IV Therapy and much more.

Social Work
Our social workers are available for help with management of the many social programs available to clients and their caregivers, as well as case management of the Wyoming State Long Term Care Waiver Program.

Sr. Care Advisor
Certified Sr. Care Advisors will assess physical, psychological, social and emotional needs and arrange for supportive services. We provide guidance to secure all eligible benefits and help navigate through the complex medical systems. Advisors will advocate and oversee that quality care is provided to the individual acting as a surrogate or liaison for our clients.

Home Health Aides
Certified nursing assistants who have completed a course of study and are registered with the state of Wyoming as Home Health Aides provide home health aid services. These services can include: bathing; dressing; assistance with mobility or transfers; meal preparation and light housekeeping.

Companion/Homemaker
These paraprofessional employees often extend the length of time people can safely remain in their homes. Services may include: companionship; meal preparation; housekeeping; grocery shopping and more. Employees are available to work a variety of hours maintaining constant supervision of patients.

Therapists
We can provide you with licensed therapists to speed the recovery from illness, injury or surgery, including:

  • Physical Therapy - to regain strength, endurance and physical movement


  • Occupational Therapy - to regain day to day skills such as bathing, dressing and feeding oneself


  • Speech Therapy - to improve swallowing and communication abilities

Home INR testing with the CoaguCheck XS System has never been easier. Patient Self Testing (PST) allows patients taking blood thinning medication to assist in their anticoagulation therapy by moving a majority of the testing from the clinical setting to the home, office or virtually anywhere they happen to be. PST can provide a more convenient and, potentially, more effective means to perform anticoagulation tests, referred to as PT/INR tests.

The HomMed Sentry Monitoring System allows you to take part in your own health care as well as have the security of knowing that clinicians monitor your vital signs daily.

During a monitoring session, the monitor collects your vital signs through a series of short, easy steps and then transmits your information to your health care provider who can assess your condition and react quickly to any concerns. The entire process takes only a few minutes, and all transmitted information is completely confidential.
