InterPATH was designed to integrate orthopaedic physicians' preferences for pre and post-surgical care with Interim HealthCare's expertise at teaching and care plan execution. This is done so that home care is an effective part of the patient's recovery from surgery, whether on a stand-alone basis or in conjunction with outpatient therapy. Home care contributes to patient compliance and patient and family satisfaction for long-term health and recovery.

Interim HomeStyles

HomeStyles was created to provide non-medical supportive services that help people remain independent and in control. It starts with a careful review of the situation and continues with an individualized plan of services. The spectrum of services with HomeStyles care is very broad and all convenient for what clients need support with typically in the home.

Here's to Your Health

Here's To Your Health is an educational monthly series we will be presenting to senior living communities and assisted living facilities. It provides valuable benefits for the residents and is consistent with our mutually common goal of maintaining independence.

Heart Connections
The Heart Connections Program was developed to help physicians with managing cardiac patients, particularly congestive heart failure patients, at home. This program is based on recognized clinical practice standards and begins at the time you make the referral with an in-home comprehensive assessment to determine the specific and individual needs of the particular patient. The patient will receive one-on-one nursing care utilizing a primary nursing concept; the same nurse from admission to discharge. They will also receive at the beginning of their home health care experience a patient teaching handbook specifically focused on education related to cardiac problems.

SureSteps

Interim HealthCare is pleased to announce SureSteps, a program developed to help significantly reduce the risk of falls among the elderly. This next generation enhancement of care focuses on medications as well as traditional strength, balance, gait and environmental safety. Our comprehensive approach includes: identifying risks through assessments, reducing risks through appropriate interventions, educating patients and families, enhanced training for our caregivers, enhanced communication with physicians.

Alzheimer's and Dementia Program

Approximately two thirds of those suffering from Alzheimer's and dementia are cared for at home by family, usually a spouse. As the disease progresses it carries with it a tremendous burden both physically and psychologically on the family members who are doing the care giving. That is why Interim HealthCare has partnered with the Alzheimer's Foundation of America (www.http://www.alzfdn.org/) to create a home health program for patients with this illness. Our specially trained field and office staff possess the high level of professional care needed for individuals suffering from Alzheimer's as well as for their caregivers. This program helps a family in delaying having to place their loved one in a long term care facility and instead, keep them in the comfort of their own home.
