Personal & Domestic Care Management

Increasingly, older adults want to continue to live in their own home regardless of a decline in their ability to manage all of the intricate details of maintaining that home. Further, the health care system has become increasingly complex and difficult to navigate. Bewildered by these challenges they may turn to their families, trust officers, lawyers or clergy for assistance. Unfortunately, adult children are often very busy with their own lives or live in a different part of the country. Also, other sources of assistance may lack the time or expertise to manage the myriad of details involved. Thatâ€™s are Custom Care: Personal & Domestic Care Management comes in.

Through the Custom Care Program we work closely with individuals, their families and others involved to get the care and assistance the client needs. Our trained professionals have years of experience in meeting both domestic and health care needs.

We offer a full range of services that include (but are not limited to):
-Assessment and evaluation of current and anticipated needs
-Custom Care plan of care development and oversight
-24/7 on-call coverage by a Care Manager
-Coordination and supervision of Home Health Aid/Live-in/Companion services
-Dietary and Shopping assistance
-Medication management programs
-Coordination of medical care and physician appointments
-Caregiver interviewing and training
-Nursing Home Advocacy Program

Further, families and other designated parties are given periodic updates as to the services in place and upcoming needs. Our goal is to allow them to be as involved as they wish while freeing them to focus more on their core businesses.
