Discharge Planners and Social Workers
Many home care patients come to Interim HealthCare following a hospital stay, needing either intermittent visit care or continuous care to continue their recovery. We work with hospital-based professionals to assure continuity of ongoing care. Intake staff in Interim HealthCare offices are skilled at understanding needs and quickly determining the capability of the office to accept and staff a pending discharge. Reliability and responsiveness are critical. Coordinating other care needs such as DME or home infusion services is a part of our service in making the discharge timely and effective.
Physicians
Physicians must be involved in home care for effectiveness and regulatory reasons. We work with local physicians to understand and meet their needs for safe, quality patient care as well as prompt response to service orders and changes. Each Interim HealthCare office also works with physicians and their office staff to provide ongoing feedback on patient status and changes.
Case Managers
Case Managers, either independent or working for a payor, often work with cases where home care can be an effective use of resources. Interim HealthCare offices understand the role and importance of Case Managers and have developed many systems to assure prompt response to inquiries, ongoing communication and, of course, good patient care.
Advisors
These days there are many people involved in making the best use of home care who do not have extensive background in home care. They might include financial advisors, legal advisors, insurance agents (especially for long term care), pastors, pharmacists, senior center staff, other health care providers and, frankly, family, friends and neighbors. Our primary objective is to be a trusted source of information for these individuals. In addition, we have programs and materials to help informal advisors be most effective and comfortable providing useful and sound advice to "clients".

Other Providers
We work with other health care providers to provide the nursing components of their services. For example, we provide the nursing support to many home infusion companies. We also provide staffing for facilities and families who want private duty nursing to supplement the care available from a facility.
Disease Management Companies
Disease management involves managing the care of patients with complex diseases such as cardiac, respiratory, diabetes, asthma and others. Much of the success of these programs comes from the program staff and educational materials. From time to time, however, a professional is needed to go to the program participant's home to assess his or her condition or provide additional educational or intervention support. Interim HealthCare nurses provide this service, as the eyes, ears and voice, for Disease Managers.
