The environment of a hospital is not always soothing, which is why many seniors choose to receive the follow up healthcare they need at home. What many people do not consider is that "home" can mean a variety of things. For some seniors, it means the house they have lived in for 50 years or more. For others, it means an assisted living or retirement community.

Our Interim Crossroads Program can provide seniors with a Care Professional who works to make this transition go as smoothly as possible. This Care Professional can greet the senior when he or se arrives and identify activities and items of interest in the new home. They can also help seniors unpack, get organized and establish a new schedule that makes the most of the community's amenities and services.
